Required Immunizations - 7th Grade

                       Immunization Rules for 7th Grade Students
 
All children entering 7th grade are required to:
 
1. Have one (1) dose of the Tdap (tetanus, diphtheria, and pertussis), and (1) dose of the meningococcal conjugate vaccine. 
 
2. Receive vaccinations and obtain the Georgia Certificate of Immunization (Form 3231) from a health care provider.
